Summary Care Records
Summary Care Records (SCR) are an electronic record of important patient information, created from GP medical records. They can be seen and used by authorised staff in other areas of the health and care system involved in the patient’s direct care.
If you wish to opt-out of SCR, please download a form below and email it to [email protected]. You can also opt-out of additional information being included in the SCR.
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
